A group of researchers at the Bourn Hall Clinic, Queensland University of Technology and the Weill Medical College of Cornell University, New York, have concluded that IVF treatment does not hasten the onset of the menopause or the severity of symptoms, having investigated the first generation of IVF patients.

When the oocyte appears one polar body/fragmented polar body, that is MII so that we can do ICSI for it. But if I observe 2 distinct polar bodies (likely) in oocyte, should I do ICSI for it? Will it be a normal embryo too?

Re: multinucleated blastomeres
I have done FISH(PGD) before on the multinucleated blastomeres on day 3, they are all fragmented without any normal signal confirmed. So most probably, they are all abnormal. But in your case, you said that you saw a single nucleus/blastomere on day 2 while multinucleated on day 1. Do you mean that the multinucleated is 2? If so, maybe the blastomere haven\'t ready to divide on day 1, so that you see single when they divided into 4 cells, just purpose not sure (although it should be divided before forming nucleus). But in the early stage, mosaicism should be considered. So I prefer to freeze it for FET cycle and select the good embryo for fresh ET.

Re: Fragmentation
Large amounts of fragmentation are usually due to contaminants, especially endotoxin. I would check to make sure the medium never gets above 37 C. I would never set a dry block to 39 to get it to rad near 37 as at the very bottom where the egg sits, it might be 40 C. Next, I would make sure that you have done appropriate QC bioassays for all contact materials. I would suspect syringes, dishes, plates, etc.

You have to check your sperm preparation. Do a second swim up and get the clean motile sperms. It seems you are doing only IVF and not ICSI. If that is the case, also check the water in the humidity chamber of your CO2 incubator. You must change the water every 10 days and the level must be maintained or else fragmentation comes. You should also know the pathology of the patient and if she is an untreated PCOS or Endometriosis, you are bound to get fertilization but embryos with fragmentation. Why don\'t you divide the oocytes, half by IVF and half by ICSI. You yourself can find the reason.

Try culturing your embryos in microdrps with 3-5 embryos per drop under oil. Also to eliminate variation in pH and control VOCs culture a dessicator filled with a medical grade trigas mixture of 6% co2,5%o2 and balence nitrogen. pH chnges and increased VOC levels are usually responsible for fragmentation in the lab. I would next examine the stimulation protocols and do a comparitive study to see if fragmentions is occuring across the board or is seen in cases with high e2 levels before HCG.Hope you find some answers.

PH is the magic kye on embryo quality. strive to reach a PH between 7.22 and 7.25 and your problem will be solved, as much there is no medium contamination. Hope this help

Re: Piping for CO2 to incubators
Copper works very well in IVF setting for CO2 delivery. Copper may be used in incubators for germicidal reasons but copper isn\'t volatile. It will not change state from a solid to a gas.

Have you considered an actual analysis of the carbon dioxide? A US EPA TO-15 VOC and TO-11 aldehydes will provide real information on the purity of the CO2. Also keep in mind that as the phase change occurs the composition of impurities will increase significantly

We have had a patient with very thin large zonas. They also had a large perivitiline space. We were very careful when denuding oocytes and if necessary left some cummulus cells in place. We found it also necessary to culture the embryos separatly as they can become stuck together. There are number of papers about zona free embryos.

Re: oocyte quality
Please check the patient\'s pathology whether she has Endometriosis or untreated PCOS. There is always asynchrony in the oocyte growth and the cohort shall carry similar quality, i.e the embryo quality suffers and implantation failure shall be there if there are oocytes of different quality. Of course along with her age, check her basal FSH , LH, Testosterone, insulin, may be you may catch the culprit hormone. Best of luck

Most of the time this event is related to one bad gonadotropin batch, provided you are sure of your lab. Can you tell us which company gonadotropin you were using with batch number.
